Selecting the right agency to provide services in your home or the home of your loved ones is an important decision. Knowing the reputation of the agency is something that needs to be considered when determining who will be sending employees into your home.

GrayHawk Home Care Solutions would like to suggest that you find out the answers to some of the following questions from the agency you are considering for the security of your home and for the safety of your loved ones.

Here are some questions you should ask when looking into an agency:

1. How long has the agency been in business?

  • GrayHawk Home Care has been in business since 2011.

2. Does the agency provide a complete package of educational material that covers such topics as types of service provided, patient rights and responsibilities, billing procedures?

  • Yes, employees are provided with written policies and procedures during orientation training.

3. Does the agency have a comprehensive written policy and procedure manual and training program for its employees?

  • Yes, employees are provided with written policies and procedures during orientation training.

4. Has the agency instituted an ongoing compliance program to ensure policies and procedures are being followed?

  • GrayHawk has an ongoing compliance regiment.

5. Does the agency verify it’s employees by thoroughly checking their references, professional licensure and affiliations and by undertaking background checks and drug screening?

  • GrayHawk has a very thorough screening process of all employees.

6. Does the field supervisor complete an initial patient evaluation in conjunction with the patient and/or family members resulting in a specific and individual care plan that is agreed to by all members involved, including the family?

  • Yes, an initial assessment is conducted and all follow-up visits are made quarterly.

7. What is the process for getting questions answered and issues resolved?

  • Whenever there would be a question or a problem, GrayHawk can be reached Monday through Friday from 8:00am to 5:00pm for resolution.

8. What is the chain of command? How responsive is the agency in resolving issues?

  • GrayHawk addresses the chain of command by attempting to resolve all issues at the lowest level possible to ensure proper resolution immediately.

9. What procedures does the agency have in place to handle emergencies?

  • All employees are instructed to call 911 first in the event of an emergency then they would contact GrayHawk.

10. How does the agency ensure patient confidentiality?

  • Patient confidentiality is addressed strongly in our employee handbook and training materials.

11. What are the billing procedures of the agency?

  • GrayHawk bills bi-weekly to ensure accuracy.

12. Ask for references to include doctors, patients and discharge planners so that you can establish an agency’s reputation in the community.

  • GrayHawk will provide references upon request.

13. Does the agency do pre-employment drug screening?
